New Patient Forms
At our dental office in Grand Rapids, MI, we try to make the entire treatment process as comfortable and easy as possible for our patients. In order to be more efficient and reduce your wait time when you come to MI Roots Family Dental, we encourage you to fill out our patient forms before your visit. Payment Options
It’s our goal to offer the highest quality care at competitive prices while also working with your budget. In order to streamline the checkout process after a visit, we accept payment in the form of cash, check, or credit card (all major credit cards EXCEPT American Express). We also offer an in-house membership program to patients who do not have dental insurance and do accept financing through CareCredit®.
